בדף הפורזץ האחורי מודבק דף בגודל 9/8.5 ס"מ, בו כתוב בכתב יד קטן מאוד את מזמור קי"ט שבתהילים. בסופו: "ו' ויגש תר"צ"?.
כריכה חצי עור. מעט נזקי עש והדבקות בעיקר בשוליים הפנימיים. מצב טוב.
